If choosing the downloadable version, please save the completed form and send it as an attachment via email to hughescully@gmail.com and to sidpriddle@outlook.com. Within 24 hours you will receive a confirmation email that your form has been  received.

1 – It is the responsibility of the Nominator to verify the accuracy and completeness of the information provided.

2 – Honors, awards, achievements and accomplishments must be included in the two page biography submitted with the nomination form.

3 – The Nomination / Selection Committee will not add to details submitted but will merely verify their authenticity and evaluate the Nominee’s overall contribution to Canadian Motorsport.

4 – Receipt of the nomination will be acknowledged.

5 – Nominations must be postmarked no later than July 29 to receive consideration by the Nomination / Selection Committee for the current year.
